Replication steps:
Open Codex
Go to Icons
Spawn Icon
Click another Codex Tab
If you go back to the icon tab in the new codex it will simply go away and fix itself. So you can't duplicate more than once that I can see.
Duplicate the codex
Resolved, closed or solved reported bugs.
